As a courtesy our office will submit claims to any insurance company not listed as long as they are not Medicare/Medicaid. Patients are required to provide all insurance and subscriber information at the time of scheduling an appointment. All coinsurance, copayments and deductibles are due the day of service. The office accepts Cash, Checks, American Express, Mastercard, Visa & Discover for payment.
